ABSTRACT

BACKGROUND: Regenerative endodontic therapy has the advantages of good biocompatibility, complete root development and high pulp vitality preservation rate. During vascular regeneration, scaffold is needed to maintain the survival of stem cells and vascular regeneration. OBJECTIVE: To analyze the effect of different bioactive scaffolds on regenerative pulp of young permanent teeth. METHODS: Totally 160 patients (168 teeth) with acute pulpitis at the age of 8 to 16 years, who were treated in the Second Hospital of Hebei Medical University from June 2016 to June 2018, were enrolled in this study. The patients were divided into traumatic group (n=92) and dental caries group (n=76) according to a cause of a disease. Each group was subdivided into mineral trioxide aggregate (MTA), iRoot BP Plus, autologous platelet-rich fibrin (PRF)+MTA and acellular dermal matrix (ADM)+MTA subgroups. The corresponding stents were used to treat regenerative pulp. At 1, 3, 6, 12 and 24 months, the tooth color, pulp vitality, and eruption height were recorded. Root canal length, root canal wall thickness, periapical shadow and apical foramen closure were compared using X-ray photography; and the success rate was calculated. This study was approved by the Ethics Committee of Second Hospital of Hebei Medical University. RESULTS AND CONCLUSION: (1) At the final follow-up in the traumatic group, tooth color, pulp vitality and eruption height were not significantly different among the four scaffold subgroups (P > 0.05). Root canal length, root canal wall thickness, periapical shadow and apical foramen occlusion were significantly different among the four scaffold subgroups (P 0.05). Among them, the effect was best in the PRF+MTA subgroup. (3) No matter traumatic group or dental caries group, the success rate was significantly different among the four scaffold subgroups (P < 0.05); and the success rate was highest in the PRF+MTA subgroup. (4) Results verified that PRF combined with MTA has excellent biological induction in the treatment of young permanent teeth with regenerative pulp. Compared with other scaffolds, it can better preserve pulp vitality, induce the formation of young permanent teeth roots and improve the success rate of treatment.

Abstract BACKGROUND: Chronic urticaria (CU) can be provoked by a wide variety of causes. Some studies suggest contact sensitization may play a role in the disease. OBJECTIVE: To investigate the incidence and distribution characteristics of allergic contact sensitization in central Chinese subjects with CU, and assess contact allergen avoidance measures in managing CU. METHODS: Patch tests were performed, following the recommended standard procedure, with 20 selected allergens, in line with the European baseline series. All subjects with positive results were prescribed appropriate avoidance measures for the sensitizing substances, while subjects with negative results served as the control group. CU severity was assessed daily from week1 to week4 and for each subject, applying the Urticaria Activity Score. RESULTS: 42.9% (233/543) of subjects with CU showed positive reactions to one or more contact allergen(s). Potassium dichromate, benzene mix and carba mix were more common in male patients, while nickel sulfate was more frequent in females. The positive rates for different allergens varied with age and occupation. The median (interquartile range) severity scores at week 1 were 20 (14-21) and 15 (14-27) for the allergen avoidance group and control group, respectively (P>0.05); and 12 (7-15) and 14 (12-17) at week 4 (P<0.001). CONCLUSION: The incidence of allergic contact sensitization in CU patients was high, and appropriate contact allergen avoidance measures benefitted CU management. Contact allergens may play a role in the pathogenic mechanism of CU and patch tests are an option for CU patients.

ABSTRACT

Wheat-dependent exercise-induced anaphylaxis (WDEIA) usually occurs 1 to 4 hours after wheat ingestion and the pathophysiology of WDEIA remains unknown. It is recommended that WDEIA patients refrain from exercise for 4 to 6 hours after wheat ingestion. We report a case of a 51-year-old man who experienced 5 anaphylaxis attacks; two of which occurred 10 to 24 hours after wheat ingestion and exercise. He has a history of chronic gastroenteritis that responds well to antihistamine drugs but not proton pump inhibitors (PPIs) and prokinetic agents. Abdominal CT results implied the possibility of superior mesenteric artery syndrome. We suggest that WDEIA occurs 6 hours after wheat ingestion in cases compounded by obstructive gastrointestinal diseases.

ABSTRACT

This study examined the possible mechanism of sublingual immunotherapy (SLIT) in the treatment of allergic asthma.Forty asthma patients allergic to dust mite were enrolled.They received SLIT with dermatophagoides farinae (Der.f) drops for one year.Thirty healthy subjects served as controls.The levels of IL-4 and IFN-γ of peripheral blood mononuclear cells (PBMCs) were determined in allergic asthma patients before and after the SLIT as well as the healthy subjects.The results showed that the level of IL-4 was substantially increased and that of IFN-γ remarkably decreased in the patients before the SLIT as compared with those in the healthy subjects (P<0.05).After the SLIT,the level of IL-4 was significantly reduced and that of IFN-γ elevated in these allergic asthma patients.It was concluded that sublingual immunotherapy is effective for patients with allergic asthma.And it may work by regulating the balance of Th1/Th2 through changing the expression of IL-4 and IFN-γ in PBMCs.
